Hyvä kirjoitus, toi sai mut käymään läpi ajatuksia viime keväältä kun käänsin omassa ajattelussani traumakinen potentiaalin nollaksi.
Alkuperäinen oma teesini oli että Remap-capissa olisi todettu että interferoni on teholtaan huonompi kuin IL-6 lääkkeet.
Perustin sen siihen että 10. huhtikuuta tuli ulos tiedote että immunemodulaatio domainissa on todettu tocilizumab ja sarilumab tehokkaiksi verratuna placeboon ja että ne ovat domainin tehokkaimat hoidot ja samalla todettu että domaini suljetaan joten ajattelin että tää oli tässä interferonin osalta ja kaikki rahankäyttö tästä eteenpäin on pelkkää negaa. Varsinkin kuin remap-capin tutkimussuunnitelman mukaan randomisointisuhdetta muutettiin suosimaan tehokkaimpia hoitoja lennosta tutkimuksen edetessä, jotta saataisiin parhaat hoidot valmiiksi ensin.
These results provide a clear indication that tocilizumab and sarilumab are equally effective for the treatment of patients with severe COVID-19, and are also the most effective interventions included in this domain. The REMAP-CAP ITSC have therefore decided to pause recruitment to this domain. A full analysis of all interventions included in this domain, including anakinra and interferon-β1a, will be completed and the results made available as soon as possible.

At that time, 6023 participants had been randomized in at least one domain in REMAP-CAP and 2274 critically ill participants had been randomized in the Immune Modulation Therapy domain (972 tocilizumab, 485 sarilumab, 378 anakinra, 21 interferon-β1a and 418 control) in 133 sites across 9 countries (Figure 1).
The effect of interferon-β1a could not be assessed with only 21 patients randomized to this intervention, as the use of corticosteroids excluded patients from this treatment. Interferon-β1a did not improve clinical outcomes in the SOLIDARITY Trial.26
Eli ei helvetti taas vuosi eteenpäin ja vieläkään eivät saaneet rekrytoitua kuin kourallisen potilaita! Eli pettymys kun remap-cap päätyi periaatteessa samaan fiaskoon kun solidaritykin, mutta mitä muutakaan olisi voinut odottaa.

3.2.2. Summary of results of interferon-β1a
It is important to note that because any clinical use of corticosteroids was an exclusion criterion for interferon-ß1a assignment, recruitment was extremely low after corticosteroids became standard of care (June 17, 2020). The full data on the interferon-ß1a treatment arm is provided in the “ITSC Secondary Analysis Report” at the end of this supplement.
A total of 21 patients were randomized to interferon-ß1a treatment. After withdrawal of consent for two patients, results are available for 19 patients. Median organ support-free days was 0 (IQR -1,13.50) for the interferon-ß1a treatment arm and 0 (IQR -1, 15) for the control group. Compared to control, the median adjusted odds ratio (primary model) for interferon-ß1a was 1.67 (95%CrI 0.49, 5.73) yielding a 79.4% posterior probability of superiority. The hospital mortality was 36.8% for interferon-ß1a and 36.9% for control. Compared with control, median adjusted odds ratio for hospital survival was 1.72 (95%CrI 0.48, 6.58) for interferon-ß1a yielding a 77.4% posterior probability of being superior to control.

Naurettavan pieni potilaspopulaatiohan toi on (19:406), mutta tilastonikkarit saivat siitä silti esille että olisi todennäköisyys että Traumakine olisi controllia parempi. Kuolleisuudessa ei näy eroa prosenteissa mutta ilmeisesti kun ottaa huomioon populaation eron niin adjusted odds ratio näyttäisi että interferonin kanssa olisi 1,72 kertainen elonjäännin etu, joka olisi suurempi kuin tutkimuksessa julkaistuilla IL-6 lääkkeillä. Mutta nyt jäitä hattuun koska potilasjoukko on niin pieni, mutta vihdoin pitkän tauon jälkeen tulee jotain dataa joka tukee että traumakinessä voisi olla jotain. Tosin tässä controlli on ilman corticosteroideja, joten oikeasti kontrolli on nykyään vähän parempi kuin tuolloin.
En usko että faronillakaan ollaan tietoisia tästä. Jos joku haluaa laittaa Jalkaselle viestiä tästä ja kysyä niin voisi olla hyvä. Nyt täytyy sanoa että onneksi laittoivat oman trialin pystyyn. Kuten sanottua ”laita WHO asialle ja tee itse perässä”.
Tässähän joutuu syömään sanojaan oikein kunnolla, mutta syön sanani nyt myös siitä että Traumakinen potentiaali on lievästi negatiivinen.
